Preoperative Symptoms:
Left mandibular ramus bony concavity
Right masseter muscle overgrowth
Surgical Plan:
Orthognathic surgery
Masseter muscle partial resection surgery
Custom-made implant surgery
Common facial asymmetry usually involves the chin deviated toward one side and occurs with malocclusion (bad bite). This gentle man, however, has a relatively rare type of facial imbalance. If you look at his face closely, his chin and lips are centered on the face but the right side face is obviously bigger than the left side. His bite is absolutely normal. The causes of the problems are yaw deviation of lower jaw, right masseter muscle hypertrophy (overgrowth) and concave shape of left lower jaw. We performed a combined surgical approach including bi-jaw orthognathic surgery (right inward with left outward), right masseter muscle partial resection (to decrease right side volume) and custom-made left ramus implant (to fill in the bony concavity). Through right side reduction and left side augmentation strategy, patient is very pleased with a much balanced face. He did not have post-operative orthodontics treatment.
