It is the skeletal bony framework covered with a soft tissue envelope that constitutes a human face. In many cases the facial aging comes from insufficient skeletal support of the maxillofacial structure and skeletal aging rather than soft tissue descent alone. Traditionally, all kinds of techniques have developed to address the droopy soft tissue while ignoring the underlying skeletal structures. As a maxillofacial surgeon, we have the privilege to alter the skeletal proportions and re-drape the aging soft tissue at the same time. It is the author’s protocol that treats the skeletal and soft-tissue aging simultaneously. Various author’s techniques will be included in this lecture.
