Orthognathic surgery with genioplasty, rhinoplasty and facial bone sculpture may be the three most powerful procedures to improve the facial aesthetics. Some other facial cosmetic procedures can be performed in the same session without problems in order to augment the cosmetic result such as blepharoplasty and facial implants. Major changes to the faces and the possible need for a new photo on their identification cards should be mentioned to the patients before surgery when combining these procedures are contemplated. This presentation aims to demonstrate some patients with dentofacial deformity who either received orthognathic surgery alone or combined other facial cosmetic procedures to achieve favorable results.

Orthognathic surgery has been through lots of improvements and breakthroughs over the last three decades. Based on cephalometric study and model surgery, we can safely predict the final location of many bony markers. However, we might somehow fail to control over the three-dimentional soft tissue changes that are affected by the surgery. If we can consider enhancing the final outcome of surgical-orthodontics treatment by performing other facial cosmetic surgical procedures either simultaneously or independently, we may greatly achieve better aesthetics and increase patients’ satisfaction. This report will focus on the incorporation of facial cosmetic surgery and orthognathic surgery and how we can shorten the overall surgical-orthodontics treatment time by segmental orthodontics and multi-segmental surgery.
