In the program, he cleverly used the analogy of a Starbucks cup lid to help everyone understand facial features such as bulges, receding chins, and protruding teeth. The theme of this seminar is "Using customized facial implants to enhance the ideality of orthognathic and osteotomy surgery." Orthognathic surgery is usually performed to correct the skeletal differences between the upper and lower jaw bones. However, the extent of craniofacial deformity is often beyond the scope of our orthognathic surgery. For example, skeletal Category III malocclusion usually involves hypoplasia of the paranasal bones, malar bones, and lower edges of the orbital bones. Le Fort I Osteotomy can usually only correct the depression of the midface below the orbital bone, and cannot improve the other underdeveloped parts. In this seminar, Dr. Hsieh demonstrates how implants in the craniofacial region can be used to enhance the overall aesthetics and improve satisfaction of orthognathic patients.

隨著矯正裝置的縮小化,越來越多人願意選擇矯正治療來改善嘴型或是臉型的問題。然而部分咬合異常是因為顏面骨發育異常所造成的。單純藉由矯正治療來拉近上下牙對咬關係不僅會犧牲牙齒的角度,同時對於臉型改善效果有限。許多患者在經歷過這類型的矯正掩飾型治療後,對於臉型仍舊不滿意而回頭尋求正顎手術的幫助。然而經歷過矯正後的牙齒,已經為了遷就咬合而造成傾斜。在手術設計時就要特別考量進去。尤其部分患者已經接受過矯正治療,不希望在正顎手術後再一次接受矯正治療,這時就需要考量到上下顎牙齒對咬的穩定度。 The indications of orthodontic camouflage treatment has been broadened with the applications of mini implants in orthodontics. In class II with maxillary vertical excess, clinician has greater ability to retract and intrude maxillary segment to maximize autorotation and advancement of mandible with mini-implants placed in the maxilla. In skeletal class III cases, camouflage may involve implants placed in the maxilla and mandible to maximize lower molar distalization and minimize upper molar extrusion and upper incisor proclination with class III elastics. Although the occlusion can be successfully neutralized through reliable anchorage device, some patients may not be satisfied with the functional and esthetic results or may even “regret” the previous decision of non-surgical camouflage approach. This presentation will focus on the orthognathic solution after orthodontic camouflage treatment.

"Lip cold and tooth death", many clinical treatments performed by dentists will significantly affect the aesthetics of the lips. For example, prosthetic dentures, dental crowns, orthodontic treatment or orthognathic surgery change the shape, position and angle of the alveolar bones and teeth, and also change the appearance and function of the lips. In fact, many hard-tissue palatal facial problems or diagnoses can be traced to the soft-tissue lips. This report discusses how clinical "vertical" changes in bite and facial shape affect the function and appearance of soft tissue (lips), and how to diagnose it? And consider the joint treatment of orthognathic surgery and orthognathic surgery to improve the position, shape and function of the patient's lips to achieve long-term and stable treatment results.

Orthognathic surgery is often regarded as a cosmetic surgery because it can significantly change the face. However, there are actually three goals when evaluating orthognathic surgery, face shape, bite, and respiratory tract. Orthognathic surgery can rotate and move the mandible forward, which can lift a large amount to widen the airway and open up multiple obstruction points at one time to treat sleep apnea. Therefore, this type of orthognathic surgery is also called upper airway lift surgery.
