Orthognathic surgery, which always has been one of many skills mastered by oral & maxillofacial surgeons, is considered a powerful aesthetic surgical procedure in the face. Also, Asian patients generally differ from Caucasians in morphology, thus require different plans, approaches, and procedures. Although we understand and manage the hard tissue with confidence, it is also true that not all of us are proficient or even interested in soft tissue management. It is a shame that not so many of us maxillofacial surgeons are publically considered as facial aesthetic experts. Sometimes we even have legal conflicts with neighboring specialties because of the range of practice. We maxillofacial surgeons share the same pride and also hardships. We often feel isolated and lonely, and that is why we need to gather together and support each other both in and out of operating rooms. Therefore, we founded the Asia- Pacific Association of Maxillofacial Aesthetic Surgeons in 2017 and held the commemorating symposium in Seoul. Many invited speakers from Taiwan, Russia, Thailand, Singapore, Japan, and Korea gave lectures about maxillofacial Aesthetic surgery. It was a great success. One of the many plans we agreed was to hold an international symposium every 2 years.

TMJ Consideration in Surgical Orthodontics & Idiopathic Condylar Resorption Orthognathic surgery is a surgical procedure for the correction of dentofacial deformities, involving the maxilla and mandible. It can restore the function and esthetics of patients by altering the spatial position of jaw bones and it also induces the structure changes to the temporomandibular joint and change the load distributed on the condyle. Condylar remodeling, a physiological balance between bone resorption and bone formation, always happens after jaw surgery. Careful preoperative evaluation of symptoms of temporomandibular disorder and the stability of TMJs is crucial to the treatment result of surgical orthodontics. In this talk, I will introduce our consideration of TMJ in jaw surgery and our treatment algorithm of severe condylar remodeling or idiopathic condylar resorption.

Comprehensive Forehead Sculpture The shape and proportion of forehead play an important role in overall facial aesthetics. The height of hairline determines the upper third length. The inclination of forehead has a direct impact on how the face looks masculine or feminine. This presentation will cover various techniques to alter the shape and inclination of the forehead. There are two basic approaches which are coronal approach and via hairline incision. Three-dimensional assisted forehead implant augmentation is frequently used in forehead sculpture. It is recommended to all facial aesthetic surgeons that upper third-forehead-temporal complex should always be treated either as a solo procedure or as an adjunctive procedure in addition to rhinoplasty, facial bone contouring surgery, genioplasty or orthognathic surgery.

Dr. Adrian M. Hsieh was invited by the Association of Aesthetic Medicine of the Republic of China on 2019/09/08 to publish the clinical results of facial bone reduction. The title of Dr. Hsieh's speech is "Complication of Facial Bone Surgery" and he discusses the clinical application of customized bone cutting guide plates to help bone cutting surgery through case studies. Preoperative 3D computerized tomography is used to understand the patient's facial bone condition. The 3D customized bone removal surgery guide can accurately remove appropriate bone fragments during surgery for the cheekbones or mandibles with different conditions on both sides. It greatly shortens the operation time, reduces the postoperative recovery period and greatly reduces postoperative discomfort. It can also improve the symmetry of both sides of the face after the operation. It can be designed according to the overall face shape and provide patients with a customized treatment plan. Through long-term advancement of maxillofacial surgery technology, Taiwan’s advanced technology in craniofacial surgery is promoted through international exchanges.

Integrative Thinking of Surgical Orthodontics From Profile, bite to Airway. How to plan? Integrated thinking in surgical correction: How to plan contours, occlusion and airway? Successful orthognathic surgery, like corrective treatment, must be based on accurate preoperative evaluation and diagnosis. In addition to the assessment of dental occlusion and facial appearance by orthodontists and orthognathic surgeons, with the vigorous development of sleep medicine in recent years, clinicians have gradually understood the impact of the maxillofacial skeleton and oral environment on respiratory function, and have also begun to pay attention to abnormal respiratory function. Advances in medical imaging technology have greatly improved the accuracy of orthognathic surgery. However, how to effectively evaluate and plan the most appropriate treatment plan for correction and surgery before surgery and 3D movement of the upper and lower jaws to improve the patient's contour, occlusion and respiratory problems is still the biggest challenge for clinicians. This report will start with the pre-treatment assessment, the formulation of the treatment plan and how to consider the patient's contour, occlusion and respiratory tract, plan a complete and stable pre-operative plan, and share treatment concepts and clinical experience with you.
