這位患者(陳醫師)本身是一位在美國執業的專業牙科醫生，諮詢過許多在美國的正顎手術醫師，但對於他們的治療計畫似乎沒有很有信心，最後經過台灣親友的推薦找到我們。
於是我們開始跨海與他在美國的矯正科醫師(Dr. Russ Trapnell)進行數次詳細的討論。
這個case的挑戰之處，除了上下顎骨的前後差距非常大之外，上顎骨的高度也嚴重不足，導致他在咬合時，非常吃力，下顎骨也會異常抖動，造成他發音、咀嚼功能的嚴重障礙，經過長期的用力過度咬合，也咬出深深的法令紋。
我們手術計畫，包含較積極的上顎延長及前移，如此一來可以給上唇及中臉更多的支撐，也可以降低下顎骨的後移量，保留了呼吸道空間，而在做上顎骨的同時，我們利用同一個傷口，做了下眼眶骨的重建，一併改善了淚溝及蘋果肌的凹陷。
術後的結果非常穩定，他感到咬合比較不吃力，下顎也不再抖動，法令紋也逐漸的淡化中，手術數週後，他如期回到美國執業，以及接受術後的牙齒矯正治療。
This patient, Dr. Chen, is a professional dentist practicing in the United States.
He had consulted several orthognathic surgeons in the U.S. but did not feel confident in their treatment plans.
Eventually, through the recommendation of friends and family in Taiwan, he reached out to us.
We began an international collaboration and held several in-depth discussions with his orthodontist in the U.S., Dr. Russ Trapnell.
The challenge in this case stemmed not only from the significant discrepancy between the upper and lower jaw positioning, but also from the severe deficiency in the height of the upper jaw.
This made biting extremely strenuous and caused abnormal tremors in the lower jaw, which led to serious impairments in speech and chewing. Over time, excessive effort in biting even resulted in deep nasolabial folds.
Our surgical plan included a more aggressive approach to lengthening and advancing the upper jaw, providing greater support to the upper lip and midface.
This also reduced the extent of lower jaw repositioning, helping to preserve airway space.
During the upper jaw surgery, we utilized the same surgical site to reconstruct the inferior orbital rims, simultaneously improving the tear troughs and volume loss in the cheek area.
Post-surgery, the results were highly stable. He reported less strain during biting, no further tremors in the lower jaw, and a gradual softening of his nasolabial folds.
A few weeks after the surgery, he returned to his practice in the U.S. as scheduled and began his post-operative orthodontic treatment.
