從咬合到臉型：正顎手術思維的轉變 這次姜厚任醫師受邀與牙科同業分享 Orthofacial Surgery 的臨床觀點。 正顎手術過去多以矯正咬合為主要目的，但臨床需求已逐漸擴大—— 患者更在意 臉部比例、軟組織變化與呼吸功能，也因此「Orthofacial」的概念受到更多討論。 正顎手術不再只是動骨頭的手術 現代手術規劃必須同時理解： • 骨骼與牙齒的移動 • 嘴唇與臉部線條的變化 • 呼吸道在術後的可能改善或調整 姜醫師在分享中以功能與美觀並重的角度切入，說明如何透過整合軟硬組織、預測手術後的變化，達到更穩定、自然、符合個人特質的結果。 希望讓更多人了解： 正顎手術是一項從功能開始、由內而外的全面性治療。

建築大師 Frank Lloyd Wright 曾說：“Form and function are one, joined in a spiritual union.”意思就是「形式」與「功能」絕非取捨關係，而是從規劃之初便彼此呼應、不可分割的整體。 隨著數位科技演進，顱顏面外科的臨床思維也不斷突破。我們從過去專注於單一部位的改善，推進到如今能善用 3D 影像、虛擬手術規劃（VSP）與 CAD/CAM 客製化設計，將咬合功能、氣道結構、顏面比例及頭顱輪廓，納入同一維度的整體思維中。 本次演講從正顎手術在阻塞型睡眠呼吸中止症（OSA）中的功能性角色出發，分享如何透過骨骼結構的精準移動，協助擴大氣道空間；並進一步探討 3D 虛擬術前規劃與客製化植入物的臨床應用，為複雜的顱顏面重建與美學設計提供毫米級的精確依據。
