建築大師 Frank Lloyd Wright 曾說：“Form and function are one, joined in a spiritual union.”意思就是「形式」與「功能」絕非取捨關係，而是從規劃之初便彼此呼應、不可分割的整體。 隨著數位科技演進，顱顏面外科的臨床思維也不斷突破。我們從過去專注於單一部位的改善，推進到如今能善用 3D 影像、虛擬手術規劃（VSP）與 CAD/CAM 客製化設計，將咬合功能、氣道結構、顏面比例及頭顱輪廓，納入同一維度的整體思維中。 本次演講從正顎手術在阻塞型睡眠呼吸中止症（OSA）中的功能性角色出發，分享如何透過骨骼結構的精準移動，協助擴大氣道空間；並進一步探討 3D 虛擬術前規劃與客製化植入物的臨床應用，為複雜的顱顏面重建與美學設計提供毫米級的精確依據。 Architect Frank Lloyd Wright once said, “Form and function are one, joined in a spiritual union.” This reflects the idea that form and function are not competing considerations, but an inseparable whole that should be considered together from the outset of planning. As digital technology continues to advance, so too does clinical thinking in craniofacial surgery. What once focused on improving a single area has evolved into a more comprehensive approach—leveraging 3D imaging, virtual surgical planning (VSP), and CAD/CAM-based customized design to bring occlusal function, airway anatomy, facial proportions, and cranial contours into one integrated framework. This lecture began with the functional role of orthognathic surgery in obstructive sleep apnea (OSA), discussing how precise skeletal repositioning may help expand the airway space. It then explored the clinical applications of 3D virtual preoperative planning and customized implants, providing millimeter-level precision for complex craniofacial reconstruction and aesthetic design.

Orthognathic surgery was once centered mainly on correcting bite problems, but clinical expectations have expanded. Patients today pay much closer attention to facial proportions, soft-tissue changes, and airway function, which is why the concept of “Orthofacial Surgery” has gained increasing attention. Orthognathic surgery is no longer just about moving bones Modern surgical planning requires an understanding of: • How the jaws and teeth will move • How the lips and facial contours will respond • How the airway may improve or change after surgery In his presentation, Dr. Chiang emphasized both functional and aesthetic perspectives, explaining how integrating soft- and hard-tissue analysis—and predicting postoperative changes—helps achieve stable, natural, and personalized results. The goal is to help more people recognize that: Orthognathic surgery is a comprehensive treatment that begins with function and transforms from the inside out.
