Actually “Orthognathic-Like Surgery” is not a academically formal term but adequately apply to the situation here. It means alternative facial skeleton contouring surgery with an orthognathic-concept (without a real orthognathic surgery) so that the surgical goal can mimic the result of an orthognathic surgery. This lady complained about her long face and clearly expressed her unwillingness of undergoing orthognathic surgery. With an orthognathic-concept in mind, our goal is to shorten her face and give her a more relaxing lip posture. We performed a combination of lower jaw contouring (reduction mandibuloplasty) and shortening with narrowing genioplasty. Note she looks more harmonious with her lips more relaxed and natural.
