The appearance characteristics of maxillary protrusion are projecting mouth/ lips, easy to have gummy smile as well as undesirable smile curve. The issue is the overgrown protruding maxilla may cover the lower teeth and orthodontic treatment alone cannot fully cover up or eliminate the effect of facial bone on smiling and appearance. Nowadays dental occlusion is not the only consideration factor to perform orthognathic surgery. Dr. Hsieh is an expert of using orthognathic surgical techniques to adjust relative position of maxilla and mandible as well as dental occlusion for an ideal facial contour. The patient shown in the photo disliked being unable to give a natural big smile. Without smile he looked stern and grim. This patient had bimaxillary protrusion with overbite, protruding mouth and mandible retrusion. Based on the results of craniofacial analysis, Dr. Hsieh performed a tailor-made orthognathic surgery to adjust and rotate the position of maxilla and mandible so the patient could smile or laugh freely without making extra efforts, which means no more mirthless smile; combined with facial contouring to correct his projecting mouth. After surgery the patient showed an attractively masculine facial contour.
