This young man was diagnosed with severe obstructive sleep apnea (OSA), with an Apnea-Hypopnea Index (AHI) of 55.8. He exhibited nearly all the associated symptoms, including loud snoring, excessive daytime sleepiness, morning headaches, difficulty concentrating, irritability, and anxiety. Like many other OSA patients, he refused to use CPAP and sought a more definitive treatment.We performed a counter-clockwise rotational Maxillomandibular Advancement (MMA) procedure, along with advancement genioplasty. In MMA, we repositioned the bones of the upper and lower jaws forward, while tightening the airway muscles (soft palate, lateral pharyngeal walls and tongue base) to reduce collapsibility and relieve airway obstruction. MMA has proven to be one of the most effective surgical approaches, addressing multi-level obstruction (including the nasopharynx, velum, lateral pharyngeal wall, and tongue base) in a single procedure. Numerous studies support that MMA for severe OSA is as effective as CPAP.Six months post-op, the patient’s AHI dropped to 3.9, and nearly all related symptoms significantly improved. Pre- and post-operative photos and CT scans demonstrated changes in facial aesthetics and an enlarged airway space.
