It is very common to have some degree of facial asymmetry in our practice. Patients may suffer from functional problems such as chewing difficulties in severe form of facial asymmetry. In this case we reported here, however, what bothers him the most is not functional at all but is that facial asymmetry affect his self-esteem and confidence especially when he smiles. After through examination including CT scan, we have noticed that the asymmetry not only involving the misalignment of the jaws and teeth but also the shape of the chin and lower border of the mandible (lower jaw). We have designed an orthognathic surgery (surgery-first approach) with chin-lower jaw contouring surgery to correct the bite and facial shape at the same time. After the surgery, we can notice the improvements of the overall facial balance, alignment of upper and lower lip and the harmonious smile.
