Abstract:
This presentation discusses the application of computer-aided system in orthognathic and facial contouring surgery. Computer-aided design and rapid prototyping techniques can be used for those patients who need cranio-maxillo-facial augmentation or defect restoration. With the computer-aided system and design presurgically, precision of surgery is improved and surgery time is reduced. Indications, limitations, pros and cons of the technique will be discussed. In general, computer-aided surgery improve the overall outcome and satisfaction for patients seeking cranio-maxillo-facial surgery.

Abstract:
正顎手術與牙齒矯正在顎顏面異常 (dentofacial deformity)患者的治療中是相輔相成的。顎面外科醫師與矯正醫師在共同計畫時,面對諸多咬合不正 (malocclusion)的病例,除了要求牙齒排列整齊、重視臉型美觀的改善外,越來越多患者要求治療的品質及效率。正顎手術與牙齒矯正相互介入的治療時機正是治療計畫中關鍵的一環。 本次報告將著重於分享矯正優先(orthodontics-first approach)、手術先行(surgery-first)或早期手術(surgery-early approach)等治療方式在臨床經驗中的應用範圍與治療案例,為患者提供有效率的治療成果。

這是全球化及資訊爆炸的時代,「時間」是每個人最珍貴的資產,也是矯正醫師在臨床治療中不可忽略的重要成本。 矯正醫師在臨床執業中,面對各式各樣咬合不正 (malocclusion)的病例, 其中往往合併高比例的顎顏面異常 (dentofacial deformity)。另外,接受齒顎矯正治療的患者除了要求一口排列整齊、健康的牙齒之餘,也越來越重視臉型美觀的改善,並且也更要求治療的品質及效率。 本次報告將著重於如何在「咬合不正合併顎顏面異常患者」的齒顎矯正治療計畫中,善用外科手術的協助,達到事半功倍、醫病雙方滿意且穩定的治療成果。

Abstract:
Aesthetic Application of Cranio-Maxillo-Facial Implants Facial implants has long been used to augment facial skeleton to enhance esthetics. It can be performed as a solitary procedure or it can be applied as complimentary procedure to orthognathic surgery or facial rejuvenative procedures. Traditionally, stocked facial implants were used coming with 2 or 3 various sizes. Over these years, the application of computer-aided design and rapid prototyping techniques is becoming relatively popular and demonstrating more advantages over traditional stocked implants. In this presentation, workflow of patient-specific-implant and its various application will be reported.

Abstract:
The Paradigm Shift of Orthognathic Surgery Kuhn 1962年發表 ”paradigm shift” 這個名詞,去描述在科學上顛覆先前認知且難以忽略的發現。當這樣的 ”shift” 出現時,所有之前對某項治療的準則都會重新被改寫,並且被新的見解及規則所取代。在正顎手術的歷史中,第一個paradigm shift出現在1967年:Hugo Obwegeser發表下顎骨上升支矢狀切開手術方式 (sagittal split ramus osteotomy/SSRO),讓正顎手術不在僅限於治療下顎突出 (prognathia),如下顎後縮(retrognathia)、顏面歪斜 (laterognathia)等都有機會以新手術方式治療。之後如David Hall, Roger West, Bruce Epker, Larry Wolford,及 William Bell等人開始上顎骨矯治手術的先河,並且在更了解顎顏面骨骼異常的3D結構之後,雙顎手術(2-jaw surgery)也逐漸取代單下顎手術成為主流。儘管近年在手術器械如內固定骨釘骨板、超音波骨刀等的發展,及3D影像技術的進步,都讓正顎手術更安全、更有效率且更穩定,但矯正或手術醫師在臨床面對的挑戰也比以往更複雜,除了咬合、顏面美觀,更多的是呼吸道及顳顎關節穩定性的考量。本次報告回顧正顎手術的歷史,展望更全方位的正顎-齒顎矯正-呼吸道-顳顎關節治療輪廓,迎接下一個paradigm shift的到來!

Abstract:
3D planning for craniofacial surgery to current aesthetic and reconstructive surgery has been increasingly developed through last decades. Orthognathic surgery for example, we can have better accuracy during the operation so that we can make a treatment plan not only for orthognathic but also for saving space to make airway reconstruction for patients suffering from OSA. For the custom-made facial implants, Virtual Planning increased fitness of facial implants. We can have better result even in the Infraorbital Implant Surgery, which has a big demand in Taiwan. It usually needs high fitness of facial implants. It has become a helpful additional tool for clinicians to optimize surgical result through simulation and increased visualization. 在這幾十年來顱顏手術電腦3D模擬系統,應用於當前的顱顏面美學和重建手術一直在不斷發展。 以正顎手術而言,電腦3D模擬具有更好的準確性,醫師能夠在制定正顎手術治療時更準確的預留空間讓呼吸道拓寬,以改善睡眠呼吸中止症患者的困擾。在客製化臉部3D植入物方面能夠提高其吻合度,例如蘋果肌部分提供客製化3D列印下眼眶骨植入手術。這通常需要非常良好的吻合性,醫師能分析事前的數據制定出精確的手術計畫,在這類型手術中得到較好的治療成果。

Abstract:
Since Dr. Altobelli first described the basic concept of computer-assisted 3D planning for craniofacial surgery 1993, its application to current aesthetic and reconstructive surgery has been increasingly developed through last decades. Different solutions for virtual craniomaxillofacial planning are basically appropriate for routine clinical use. Orthognathic surgery, facial bone sculpture, genioplasty and custom-made facial implants are some of the most commonly applications of virtual planning to the facial aesthetic surgeon. Compared to conventional methods, virtual planning has more advantages such as better accuracy, better communication with orthodontists, decreased operation time, decreased complication and increased fitness of facial implants. It has become a helpful additional tool for clinicians to optimize surgical result through simulation and increased visualization. 自從1993年Altobelli首先提出了電腦系統輔助3D規劃顱顏面手術的基本概念後,其應用於目前的顱顏面美學和重建手術在這幾十年來一直在不斷發展。透過電腦模擬系統制定不同的治療計劃已普遍的存在於臨床應用。正顎手術、削骨手術、截骨手術和臉型客製化植入物手術是顱顏外科醫師最常應用到的範疇。與常規方法相比,電腦模擬具有更好的準確性,能與矯正醫師達到更良好的溝通,減少手術時間,減少併發症和提高臉部植入物的吻合度等優點。有了治療計畫模擬與提高多方視角的好處,電腦模擬系統已成為臨床醫師有效提升手術成果的附加工具。

In general, computer-aided surgery improve the overall outcome and satisfaction for patients seeking cranio-maxillo-facial surgery. 本次演講的主題主要討論電腦科技在「正顎手術」與「臉部輪廓手術」中的應用。電腦科技的「設計」與「3D列印」兩項技術能被用於解決「顏面或顱部需要植入物」或是「顏面受損需要填補」的病患。透過電腦輔助系統與術前設計,能夠有效提高手術的精準度並大幅縮短手術時間。後續我們將對於電腦科技在應用上需適應改變的地方、應用的局限性及各項利弊等三方面做討論。整體而言,電腦科技的應用有效地提升整體的手術成果與病患的滿意程度。
